Nurse Practitioner Support & Alignment Network

The National Nurse-Led Care Consortium and the American Association of Nurse Practitioners (AANP) have joined together to create the Nurse Practitioner Support & Alignment Network (NP SAN). The NP SAN is a grant-funded to program to provide NPs and their practices with the tools and education they need to prepare for changes coming to healthcare reimbursement. As clinical practices move from fee-for-service to new, value-based payment methodologies they will need the skills and tools to provide patient-centered, high-quality, sustainable services. NPs are the future of primary care and the continued sustainability and success of their practices are critical to improving the health of the nation.

Clinicians who join our NP SAN Email Listserv can receive:

  • Linkage to free, peer-based practice transformation networks.
  • Education and training to support core competencies necessary for value-based reimbursement.
  • Free continuing education (CE) and clinical training programs.

The NP SAN is a grant-funded program of the Transforming Clinical Practice Initiative (TCPi). The TCPi is the federal government’s largest-ever investment in peer-based learning networks to support quality improvement and practice redesign. Involvement in the NP SAN does not require larger participation in the TCPi.

TCPI Power Packs

TCPI Power Packs

As the only professional nursing organization involved in the Transforming Clinical Practice Initiative (TCPI) we were part of a collective of 10 professional associations charged to assist over 140,000 clinicians in improving how care is delivered by providing technical assistance support for integrating quality and process improvements, and by building on and spreading existing change methodologies, practice transformation tools, published literature, and technical assistance programs.

In collaboration with other SANs we developed a number of Power Packs to spread the transformational work undertaken by clinicians and practices nationally.

Each Power Pack presents a specific service delivery challenge and the steps taken by the spotlighted practice to address the challenge. Power Packs provide a set of change tactics and resources from the SANs and other professional associations that clinicians can use to address a similar challenge within their own practices.
